Does anyone know of this?

Does anyone know of any programs that can help me live independently? I'm 26 I'm still living at my mom's house. I want to live independently. I have disabilities I have epilepsy which is under control I also have a hearing loss. I heard of programs that can help people with disabilities live independently. I want other people to help me besides my mom. I want to get on one. Does anyone know of any?
